Privacy Policy
This notice explains how information is handled when you use the iON Health + Performance website, contact us or subscribe to The Signal. It also explains how to find the privacy information that applies to your appointments and other services.
Who to contact
This website is operated by Holtom Chiropractic Professional Corporation. For website privacy questions, access or correction requests, or concerns about how your information has been handled, contact admin@ionhealthperformance.com, call 519-340-0358, or write to Dr. Ben Holtom at 449 Laird Rd, Unit 13, Guelph, Ontario N1G 4W1. Please send a general description of your request rather than medical records or other sensitive information by ordinary email.
The Orangeville clinic operates through a separate professional corporation under Sam MacDonald. Patient records are the responsibility of the practice or practitioner providing your care, rather than the website as a whole. For an Orangeville record request, contact that clinic through the Orangeville location page. If you are unsure who holds your record, our team can help direct your request.
Website enquiries and coaching applications
Our forms collect the contact details and general information you choose to provide so we can respond, discuss a service or direct your enquiry. Contact enquiries are sent to our administrative inbox; Performance Health Coaching applications are sent to Dr. Ben Holtom. Relevant team members may receive information needed to respond to your request.
Netlify hosts this website and processes website form submissions. Form notifications are delivered through email, which we use through Google Workspace. These providers may process information outside Canada, including in the United States. Please do not submit diagnoses, lab reports, detailed health histories or urgent medical concerns through website forms. Your practitioner can explain how to provide clinical information through the appropriate service platform.
The service finder
The service finder uses your selections to suggest a starting point. Your answers remain in your browser's memory while you use it. They are not sent to iON, saved as a clinical record or transferred to Jane. Opening a booking link takes you to Jane, where its account and booking processes apply.
Appointments and health records
We use Jane for scheduling, billing and clinical records. The practice or practitioner responsible for your care provides the applicable consent and privacy information. Ask that practice about access to your record, corrections, sharing and retention. Jane's privacy notice describes its role as a service provider.
Sharing the iON name does not give every practitioner or location unrestricted access to your health information. Access and disclosure must have an appropriate purpose and comply with applicable consent and legal requirements. Ontario's Personal Health Information Protection Act applies where relevant to your care.
Training, coaching and other services
Our training and coaching services use Trainerize. Information you provide there may include training goals, workouts, nutrition, habits and progress. Your coach can explain the information needed for your program and who can access it.
Links to NiaHealth, Fullscript and other external services take you to providers with their own privacy notices, account processes and consent arrangements. Review these before providing information. Ask your practitioner what information they will receive through a referral or account connection. Do not send lab reports or other clinical records through this website.
iON earns income from purchases through its Fullscript dispensary. You may purchase elsewhere without affecting your care. A purchase or appointment does not by itself subscribe you to The Signal.
The Signal
When you subscribe, your email address, optional first name and newsletter preference are sent to Mailchimp. We use them to manage your subscription and send The Signal. The website form selects The Signal; it does not select a separate monthly clinic newsletter.
Mailchimp processes subscription information and delivery records and may record email opens and link clicks to help us understand newsletter engagement. Its services involve processing outside Canada. You can unsubscribe using the link in an email or contact us for help. Unsubscribing from marketing does not cancel appointments or remove records that must be retained for another purpose. See Mailchimp's privacy information.
Website technology
Our hosting provider processes technical information, such as IP addresses, browser details and request logs, to deliver and protect the website. Embedded maps and other external content may connect your browser to their providers. External services have their own privacy practices.
Google Analytics is optional and loads only after you choose Allow analytics. It helps us understand visits to our public information pages, including page views, approximate location, device and browser information, and visit timing. Google processes this information and uses first-party analytics cookies. Processing may take place outside Canada. We do not use advertising pixels, Google signals, advertising personalization or user IDs in this website implementation.
Our contact form, coaching application, service finder and confirmation pages do not load analytics, even if you have opted in. We do not send form contents, service-finder answers, URL query strings, URL fragments or referring-page addresses to analytics. Automatic form, outbound-link, site-search, video and scroll measurement is disabled.
You can choose No thanks without affecting website services, or change your decision through Privacy choices at the bottom of any page. We save that choice in your browser for 180 days. Withdrawing consent stops future analytics collection and removes this site's Google Analytics cookies; it does not erase information already received by Google. Analytics cookies are configured to expire within 180 days. Blocking browser storage keeps analytics off. See Google's privacy policy.
Your choices and requests
You can ask about information held about you, how it is used, and how to request access or correction. We may need to verify your identity before releasing information. You may also withdraw consent where applicable; we will explain any effect on a requested service. Some information must be retained under professional or legal requirements and cannot be deleted on request.
Website enquiries, subscriptions, purchases and clinical records have different retention needs. Our policy is to retain information for the purpose for which it was collected and applicable legal or professional requirements, restrict access to those who need it, and arrange secure disposal when it is no longer needed. Contact the responsible practice for the retention rules that apply to your clinical record.
We use account access controls and service-provider safeguards appropriate to the information being handled. No online service can guarantee absolute security. Please contact us promptly if you suspect a privacy incident involving our website or services.
Questions, complaints and changes
Contact us using the details above if you have a concern. You can also contact the Information and Privacy Commissioner of Ontario about Ontario health privacy matters or the Office of the Privacy Commissioner of Canada about matters within its jurisdiction.
Information processed outside Canada may be subject to the laws and lawful access requirements of the country where it is processed. Ask us about the providers used for your service. We will update the effective date of this notice when our website practices change and provide additional notice or obtain consent where required.
